Default sustiva and depression

I've been on sustiva for three weeks now. After about a week, I started feeling severely depressed. I have had depression before about my diagnosis and other things but this depression was different. It isn't really about anything in general, but is an overwhelming feeling of despair that makes me choke and sob and feel like I can't breathe. My doctor was very dismissive of depression as a side-effect of sustiva. She said "Of course, you are depressed. You have AIDS." It was only a matter of time before I became severely depressed she says. However, this despair began at the same time as I started taking Sustiva and is only getting worse. I don't want to be on a PI because of fear of lipodistrophy. Does anyone know any substitute for Sustiva for people with depression from it?
